To simplify the treatment process for you, we provide you with all the checklists for the perfect preparation of your dialysis.

You want to register for vacation dialysis? Send us the completed registration form ‘Registration for vacation dialysis’ by e-mail or mail.

Furthermore, we ask you to have the medical questionnaire filled out by your permanent treatment dialysis institute and send it to us.

Checklist for vacation dialysis

Our goal is to make your stay at the Holiday Dialysis Institute as pleasant as possible. For this purpose, we have summarized a short checklist of all the necessary information for your treatment at Vacation Dialysis Dr. Jilly:

  • Dialysis protocol incl. anticoagulation and dialysis access r./li. as well as the current dry weight indicating the residual function and the average and/or max. UF rate.

  • Last doctor’s letter / diagnoses

  • Virology (recent) HIV; Hepatitis marker (max. 2 months old).

  • current vaccination status

  • Blood type

  • The current Covid measures are constantly being adapted and are therefore subject to change at any time.

  • Last lab (BB; electrolytes; Astrup, etc.).

  • Bring specific medications (refrigerated if needed) (iron, erythropoietins, catheter plugs (Aktilyse, Taurolock, etc.); explicit heparins if enoxaparin (Inhixa, Lovenox) cannot be used;

  • European Health Insurance Card and ID Card

  • Please provide us with a contact number of you and your travel companion where we can reach you, as well as the name and address of your accommodation.
